Integration of care – the way forward

During the 61st EASD Annual Meeting on September 18, IDF Europe will hold a symposium titled “Integration of care – the way forward”.

Building on last year’s symposium on personalisation of care, this year’s session will explore the urgent need to advance integrated diabetes care across Europe. The programme will showcase practical examples of integrated care models implemented in various European countries, highlighting successes and lessons learned.

Key experts will engage in discussions on ongoing efforts to address the fragmentation preventing the wider implementation of integrated care models across Europe, examining the main enablers and barriers to progress. The voice of people living with diabetes will be at the heart of the conversation. They will share their experiences and expertise, explaining why integrated care is essential to achieving better health outcomes and leading long, fulfilling lives.

Dr Benedetta Armocida (Istituto Superiore di Sanità – ISS, Italy), Coordinator of JACARDI, will take part in a panel discussion on “Overcoming fragmentation: enablers, barriers, and the path forward to integrated care”. She will be joined by Ms Cajsa Lindberg, T1D advocate, Prof Martin Clodi, Head of the Internal Medicine Department at St John’s Hospital (Austria), and Prof João Raposo, IDF Europe Chair-Elect.
